Veterinary Behavior of Indiana Puppy Preschool Liability Waiver and Agreement



ASSUMPTION OF RISK. The undersigned (“you” or “Participant”) acknowledges that he/she is voluntarily participating in dog training provided by Veterinary Behavior of Indiana (VBI) with knowledge of the potential dangers. Participant represents that/she is the owner or guardian of the dog(s) identified in this Agreement. You acknowledge that dogs are unpredictable animals with their own will and drives, and therefore, regardless of training, handling, or environmental circumstance, VBI cannot protect against every potential injury, risk, or accident to you, to other persons participating in the training or on the property where the training is taking place, or to dogs. *

CLIENT'S REPRESENTATIONS. You represent and affirm that:

You are at least 18 years old and competent to agree to be bound by the terms of this agreement.
You are the lawful owner of the Dog.
The Dog is under the care of a licensed veterinarian and up-to-date on all puppy vaccinations and deworming appropriate for their age.
The Dog is in good health and in a physical condition for the purposes of participating in the training exercises being taught.
All of the information you provided to VBI during the intake and registration process, and all other written and oral information provided to VBI concerning you, the Dog, and your household, is true, complete, and correct in all respects.

PHOTO RELEASE. By signing below, you give VBI permission to publish in print, electronic, or video format the likeness or image of you and/or your dog. You release all claims against VBI with respect to privacy rights, publicity rights, copyright ownership and publication, including any claim for compensation related to the use of the materials. *

CANCELLATION FEE. Makeup classes are not available, but the owner can schedule a 15-minute phone call with the instructor to go over missed information/exercises. *

CLASS RULES

Up to 2 people can accompany each puppy to class. *Children under 10 years old are NOT allowed in class.

All puppies must be between the ages of 7-18 weeks old at the start of the 5-week class.
Puppies must have a minimum of one set of vaccines at least 7 days prior to the first class and a first deworming. * They must be kept up-to-date on vaccines and dewormer throughout the 5-week session.
Participants must not take their puppies to public places outside of class where a lot of dogs congregate (examples include dog parks, public spaces).
Puppies must wear a leash and collar and/or harness. Prong collars, shock collars, and choke chains are not acceptable.
Owners will be responsible for bringing treats for their puppy for use during class. Recommendations for treats will be discussed at the orientation. Treat pouches are strongly recommended.

KNOWING AND VOLUNTARY EXECUTION. By signing below, you acknowledge that you have carefully read this waiver and agreement, understand its contents, and agree to be bound by its terms. *
